Consent Form for Story Publication
Kourtney Lynn LLC is preparing its next publishing project, “...An Anthology of Stories from Foster Youth.” Foster youth will have the opportunity to submit their creative writing and artwork to be considered for publication in this multi-volume series.
Procedures
Participants will be asked to submit an original short story, poem, or drawing. Submissions must not include any identifying information about themselves or others. Editors will review all submissions and select stories to potentially include in the book. Any identifying details will be removed. All work will be lightly edited for legibility. A legal guardian must sign this form granting permission for a youth’s work to be published. All submitted work must include the author/artist’s name and age, matching the permission form on the back.
Benefits
Participants will have the chance to be published, providing a creative outlet for self-expression. The final published book will help raise awareness about the experiences of children in foster care. But even better, the stories may reach others “still consumed by the fire.”
Risks
There are minimal risks associated with participating. However, due to the personal nature of these stories, there is a small chance of emotional discomfort when sharing one’s experiences. Youth are encouraged to speak with their counselors if they have any concerns.
Confidentiality
Legal names and identifying information will be removed from any published work. Stories will not include real names or personal details.
Voluntary Participation
Participation is completely voluntary. Permission may be withdrawn at any time prior to publication.
Consent
By signing below, you confirm that you have read and understood the above information and that you voluntarily agree to participate under the described conditions. You also confirm that the submitted story or poem is your original work. Upon publication, the submitted work becomes the property of Kourtney Lynn LLC, which will continue to market the book and manage future volumes for other participants.
